Please be sure to bring the following to your initial appointment. If you have any questions, feel free to speak to our staff by calling 202-496-9181.

• A photo identification card

• Your medical insurance cards

• A current list of all of your medications and the usage instructions.  Please include any over the counter supplements in this list.

• Your eyeglasses and/or

• Your contact lens prescription and/or the packages that your contact lenses come in

• Registration forms and eye health questionnaire.  To save time at your first appointment, you may print out and complete our introductory paperwork ahead of time. You can download these documents below. Please give them to the receptionist when you check-in. If you are unable to complete them in advance, you may do so in our office before your appointment
